In general, you have three options:
- use already implemented mechanics, changing only the database
- use scripts and change the database
- hardcode anything directly in the code, create your own custom flags for items and custom aura effects. Anything.
So I think your question is "how to do this with minimal changes to the code and database."
My answer: I can't say for sure, I haven't done this for a long time.
But I can say for sure that I recreated all heroic items based on inheritable items and gave them the ability to adapt to any level.
And I also reworked the spells of these items so that their power also changed depending on the player's level.
I also created custom items with random stats that were applied randomly in combat in the form of battle hardening (but for this I had to change the influence of these stats so that they were useful for any class to one degree or another)
All these changes except heroic items required intervention in the dbc and server code. Heroic items - only DBC
