Hopefully. I still barely understand it, I can't even cap my CPU cores and run into server issues for no apparent reason. I've looked at CPU, RAM, disk etc and there's no indication as to why it happens. There's even very little to no wait time.

I briefly looked into this. For world updates, I would say that half of the CPU load comes from handling spells (including auras). If you enable the playerbots module, the CPU load will roughly double (which also affects the world update time aka diff), due to the additional AI handling for those bots.

Have you looked at what would be required to split the world server into multiple processes or threads for every world, including instances?

You can split maps into different threads but you still can't run a map in multiple threads
