so you can use one authserver to log into multiple realms of different versions. and because of that you need extra checks to adjust the packet structure

That's the point, multiprocess where each process can't try to keep up as much as it can with world updates

If it's a seperate process you just decoupled it from the other map updates, so it doesn't block the other maps world update

If you really want to scale to the very high player counts I think you need to process regions on the map over multiple threads, and suffer from the issues that it brings. Commonly done by trying to divide zones into regions that you process multi-threaded, and starting areas + cities. You have a whole world of complexity doing escorts + kiting of mobs etc over these regions, but it's do'able.
