if we're on this subject - the server needs to do a better job of actually making sure transactions get executed.
lots of places it just hopes for the best and everything falls apart under a heavy loaded database. tons of item duplicates, for example, as desyncs occur when states aren't always being saved properly.

each instance is reset with 1 transaction, with multiple db threads executing them and fighting over the same tables

I have an issue where under a high load items are not always getting persisted correctly, they disappear from the player after changing map or relogging. I'm having a lot of fun trying to track that bug down.
