Yeah exactly.
For example if you're adding a custom item, you make it, plug it into item_template in DB.
But you also need to add it to item.dbc
And then make a custom patch and put the item.dbc in there
And add it on wow client
Otherwise it won't work correct in game

You can find guides on most things online.
Most things dbc wise are same for trinity so even if its a guide for trinity it will show you how

update: I actually tested it and adding an achievement to the db works without editing anything else
