SQL it's the easiest part of the adventure in my opnion. Learning to use modules / code cpp, change the core to your need, even using lua for certains that can be more efficient in programming and dbc changes (if you want custom content)

But i would suggest you Steve, before adventuring in programming the imposible at the first (i mean impossible when youre not familiar with the core, friend here: https://www.azerothcore.org/doxygen/) Screw around with out of box experience and just change settings around (in the world config). Add modules and see how taht experience fits you, screw around with Keira3 allows you change a lot of things (it makes SQL stuff easier to read, like properities of a creature or items), Make your custom npc with keira3! Modify modules or make your own modules. or do small tweaks to the core (that suit you).

DBCs are easy, just get them from either the server or extract them from the client, edit them and then create the MPQ archive and add it to the client.

Example to what revision answered you. Custom items if has a new unique item number, the client wont load many aspects of it visually. You grab that information and add it into a DBC that gets patched into a "zip" readable equivalent file (called MPQ) for world of warcraft that the client will know "oh that's a new item"
