Looked through it now and the documentation, quite confusing.
Edit: figured out this part, One new entry for each achievement.
Also not sure what to put in type, value1 and value2 fields.

**criteria_id** points to achievement_criteria.dbc ID
**type** can be 0 in your case probably
**value1** is achievement points amount
