At least in 3.3.5 (but probably others) Spell.dbc field "SpellVisual" -> SpellVisual.dbc field "ID". Then SpellVisual.dbc field "StateKit" -> SpellVisualKit.dbc field "ID". Then SpellVisualKit.dbc field "ChestEffect" (I think for that specific effect) -> SpellVisualEffectName.dbc field "ID". Then you change the model reference in the filename column

I haven't done form replacements so I'm not 100% sure. If I were to guess, it's probably a row in SpellShapeshiftForm.dbc, and then linking column CreatureDisplayID to CreatureDisplayInfo.dbc. That seems to be where other forms are handled
