Make sure that you fill the info in creature_model_info, as you did.
But in creature_template_model
> Verified build > 12340
Attach the modelID to a creature and reboot
Adding this value solved the issue after restarting the server.

This needs to be done for TC too. If only the dbc have the data, morphing works but creatures won't find the model.
Add the entries to your creature_model_info DB table as well and they will show up.

Edit, just reread your initial post and noticed you already did that. My bad.
Did you by chance mix up the ids? The table is sadly named confusingly and requires the CreateDisplayInfo.dbc Id instead of CreatureModelData.dbc.

Nope, I dunno whats wrong with it. I've done it just like any other. It just refuses to work on a creature. Works when you morph yourself just fine.
