However, the client had trouble loading the file from the doodad, so I modified the regular weapon version using 010 Editor to ensure the texture is correct. The only issue I have right now is that I used the head attachment ID (ID 20), but the skull moves along with the player's axis. I need it to move independently around the player without being affected by the player's orientation, if that makes sense. Do you have any suggestions for a different attachment ID, or does the retail client support more features that might make this possible?

The doodad will work you just gotta rename the m2 a small bit so it thinks its a different file in a sense. Im not sure if there are other attachments that’ll work better might have to play with it. This version of doing things above is like the hackfix way of doing it in 3.3.5 unless you make fancy scripts to force load things
