Hi, I'm currently experimenting with retroporting a Warlock artifact weapon from Legion into WotLK. Specifically, I'm working on the offhand item Skull of the Man'ari (see WoW.export). The unique feature of this offhand, or rather the M2 file, is that the skull doesn't sit in the player's hand, but instead floats around them. This part is working as you can see in the second image.
However, this only happens when the offhand is actively drawn in-game. What I'm trying to achieve (and how its working in retail) is for the skull to float around the player at all times, as long as the offhand is equipped, without needing to draw it.
There is also a doodad version of the skull (though I’m not familiar with how doodads work). Has anyone successfully achieved this effect, or is it simply not possible to implement in WotLK?

A long time ago I ported the artifact weapons, including this skull, and it's the same for me.
Maybe the player will get an aura so that the skull is always there when the item is equipped, but that in turn means that when you draw the weapon, two skulls will fly around. Therefore, I don't know of a solution.
