You need to make a second race for worgen. So like human_w (or whatever) and the alter form spell flips the model from worgen to human_w .... there is a lot more to it but that's the general idea.

I guess you'd need to alter the UI model to have a new attachment and add a new model, maybe check the difference between when the second model was added and before
