Most of the annoyance of voice chat can be solved for.
Call of Duty has such a notoriously bad voice chat experience because it's Voice-Activation (controllers don't have enough dedicated buttons for Push-To-Talk, nor would many console players use it if it supported it) by default. Their problems are compounded by garbage microphones, and the common issue of game audio feedback since they listen to the game on a television.
I think along with the differences between Console/PC audio setups, along with a PTT setting by default, you could mitigate many of the common issues with voice chat.
Obviously you'd still want options to mute or disable the feature entirely, but I really think the concerns are overblown.
