If you're using DBC to create the npc you can discard those. It only needs it for the skin afaict

helmet and cape displayIDs are correctly put in the creaturedisplayextra file, but do not show ingame, the rest of the gear does show up correctly

attachements(capes, shoulders, helmets..) are not part of the NPC/player texture, they are separate models that use their own textures, either hardcoded or from itemdisplayinfo
