when building a random item generator, do you usually just create thousands of items, assign random stats to them and then just have the random item generator select a random one of the pre-build items, or is there a better approach to that?

I'd filter it a bit. You usually don't need cloth items with e.g. strength if you don't have any custom classes that would need that.
I'd probably start with writing a stat weight formula, then specify the filter list to not generate nonsense items and then generate them

And if you want some special items like that for e.g. a specific boss or smth, create them by hand

interesting, but it would then still be thousands of item_templates that get randomly chosen right?

Depends on your plans and how you build it. Makes no sense to generate 100 level 37 cloth items that offer 50 int with every single, available off-stat if no spec will ever have a use for that or you don't have enough content to let that stuff drop

Especially for e.g. green items you probably can just use the item suffix system that already exists: https://wowpedia.fandom.com/wiki/Item_suffix
[Embed: Item suffix]
An Item suffix is a dedicated suffix given to equippable items that feature randomized secondary stat ratings upon generation. These suffixes indicate which secondary stats are present on an item.
Items of this type, like others, have a base item name. Unlike other items however, their name is followed by a suffix. These suffixes are similar to...
