Might be something with azerothcore im not 100% sure here then typically ive not had much problems with doing it that method but i typically use trinitycore. Maybe someone else here has expierence or you could try the azerothcore discord.
Edit: and put upgraded map.dbc and areatable.dbc to patch-enus-4
Did you only put the dbcs into the patch-enus-4? or the entire map files? If just the dbcs that is likely the problem if you did everything then what i said above is likely going to help you more.
