there is no different version for left/right hand simply becase blizz can now flip it with a correct flag
the only way you can do is to downport it and edit with wbs

so I just get m2 files of the spells and do the exactly the same process as backporting creatures?

Process is slightly different depending from what xpac u port from but thats essentially it. Pretty sure tehre are some guides scattered out there on how to do it

Do I get just copy/paster structure from the MPQ that filename is taken from and not relevant to the current MPQ to which filename is being copied over to?

there is no "folder structure" inside the mpq, the folders are just part of the filename with \ characters in them
