due to GUIDs, are item entries incapable of actually utilizing 32 bits? or is the 35~ ish million limit on item entry not a fundamental limitation? (ie just some annoying code somewhere, like item appearance id limited to 16 bit because blizz do `movzx` instead of `mov` for no reason)

GUIDs definitely fundamentally limit mapids to 16 bits though right
