Mmaps may appear to be fine but if you haven't already I would build a recast debug mesh and view that just to make sure that what it is generating does actually look correct. That's usually the first point when debugging vmap/mmap issues and helps to narrow down the issue.
Which version of Noggit was used can also impact it, on one old map I had to get a specific old version of noggit, 'touch' all WMOs/M2s placed and then save it, then resave the map in a new version of noggit and it fixed the issues.
It's possible your map is not being picked up correctly in the extractors too, make sure you check in the program outputs for your map being processed without errors.

Thank you, will go through these steps and verify. Just fyi, it's a WMO contained in a wdt, so a map without terrain.

I haven't done any maps without terrain but I would still recommend starting with checking the mmap debug output and the extractor logs. Hopefully you should be able to spot from that something is not correct, then based on that it is easier to point a finger at what might be the problem
