I'm having ideas for something I want to try, but it's pretty much reliant on that being something I can make work, even if I have to hackfix it in.

no idea about wotlk but in master you will most likely have to play with hotfixing npcmodelitemslotdisplayinfo and creaturedisplayinfoextra

Yes, I use 3.3.5 and creaturedisplayinfoextra doesn't get me anywhere because it assumes that you have a backend texture and a naked model, which you then dress with items. because creaturedisplayinfoextra doesn't work together with texture variations etc.

it's the same just without DBC editing. is essentially the creaturedisplayextra.dbc as a database and also requires a model that is naked
