I did a drastic change, and swapped the ChrModelId in ChrCustomizationOption from Blood Elf Female to Human Female, and the same for Human Female to Blood Elf Female, just to check out the result. From before, I have enabled the NPC-only eyes, which includes the drac'thyr eyes and some of the elemental eyes. In addition, from the beginning I had created a custom eye color choice for Blood Elf Female that is using a human female texture. This did not work on my blood elf.

The result is that none of the official eyes are working, however the elemental eyes that has a shared texture and is already working across all races seems to still work. The drac'thyr eyes does not work, but I am pretty sure this is because they are related to "Eye Style" option, and will start work if I swap this too.
Funny enough though, the choice that I added manually that did not work for my blood elf has started working when the option was swapped over to human.
Are you sure there couldn't be anything else fishy going on?

Just to help support my suspicion about eye textures, I can do the very same with skin colors:

Its only eyes I haven't been able to do this to, and I have tried pretty much every table along the way. Choices, elements, material tables, etc..
