I tried adding an items.dbc with the single entry and then creating patch-4.mpq to input it, restarted everything, and it produces the same result. Is there really no way to create patches without breaking prior content?

Take the whole old file, ADD your entry, so it contains all old stuff and your single new one too

I know that would work. But shouldn't I be able to append the change to an existing MPQ or have it not overwrite old mpq data if I use a different dbc name and compile that into the patch-4?

You can use the original Item.dbc file and add the new entry.

I don't wanna include redundant data in a patch file. Is there no way to append a new "Items.dbc" with any custom items; considering the original file and items are called with "Item.dbc"?
