New items created through a profession. They socket gems etc into the item, then relog, and the item is gone

I have tried a lot to reproduce it, and cannot. But in real player loads, it does happen fairly regularly, and a big source of player frustration. But all my focus was on verifying interactions with instance_template, I need to dig further into character_inventory as suggested by Sharuen and verify when GUIDs are being generated and that it is thread safe.

my vote is on some query error and the transaction getting dropped and not a async order issue

side story about missing items: i've had very weird user reports too about losing items. turns out client deletes item cache entries when you change the ID of an item
