I think the biggest problem is that you are not supposed to be allowed to fly in Azeroth on this patch

IIRC AzerothCore has an addon for this, but I don't think that's a thing here.

But that would allow players to fly on Azeroth in general - not only when dead. So careful with that, since there are ugly looking "dead zones" that a non-flying player would never reach - but a flying player could.

If you want to spawn dead players right at the instance entrance, you could create a graveyard there and make people get to this graveyard when dying in a certain instance.
So, many workarounds are possible - whatever fits your needs.
