Iirc the Disciple of Naralex in the Wailing Caverns should be healable.
Maybe that's a start to look how it's done?

I've already tested the flags and they don't work. I have a quest boss that also causes damage to my NPC and that works perfectly, but unfortunately it cannot be healed. NPC has faction 1665 player friendly for creatures vulnerable to attack.

instead of straight up rejecting what im trying to say thinking that it cant be pvpflags because the npc isnt for pvp, just follow the logic from WorldObject::IsValidAssistTarget (no, editing this function on server will not do anything - client has the same code that prevents targeting BEFORE it sends a cast spell packet)
