You are unfriendly with all the horde factions of the various capital cities. The only one the DK ain’t unfriendly with is one called ‘Horde’ and the ID of is used for Thrall so the DK can complete his quest xD

Then after completing that quest, Acherus is set to normal world phase and you are given Horde faction reputation with all the cities.

But I specifically wanted Thrall and Varian to have the Orgrimmar and Stormwind reputations, not Horde and Alliance.

The quest you completed before the last one. You could set it to reward just Orgrimmar reputation. But then you would be spat on and thrown fruit on by friendly and talkable NPCs xD
