or appending some naming after the rev_xxxx_my-fix-name.sql in order to quickly see what fix is about in the folder

Idk but I don't think it would be merged. The commit message should suffice for that information.
Also small PRs are preferred over large refactors involving multiple files. So I can't see the usecase really
