You know that the best way to find out is to remove everything custom, absolutely everything including modules, core patches, database edits, and test it again. If it works then you'd have to add each module one by one and test it to find the culprit before reporting it on the modules github repository. Also, .die doesn't give you loot unless enabled in the config in case you thought you could test it that way.

Also, just to prove that I'm not saying you're wrong - I just went into 25-man ICC and used .damage to kill everything and absolutely nothing (not bosses, not trash) dropped any loot.

Nevermind, some trash do drop loot - just not many of them.
