Very low thread numbers are a known cause, i take it you have more than 2 threads running?
If so i can only think of a module.

Ah! No mod-eluna here installed though. I see the CPU usage jumping from core to core, 2 are at 100% (two realm) and the other 6 are at 5%). Sometimes there is a 3rd cpu getting a better load, and then back to <5
