If it's just money on the creature, it's instant. Sometimes it's instant with items, perhaps items I've seen before, but it seems like it does it with repeated items too.
I did delete my client's cache a while back, but it's repopulated now.

Yes, that's what they do. Initially they start where DK spawn in for the first time and fight like jackals and scream for heals. It's quite assuming to watch at first.
