I know there's ways with the item exchanger module through dialog options, but as a pure and simple vendor?

That's correct. Only gold can be adjusted. The rest is custom scripting.

It's how blizz designed it back in the days without considering anyone being allowed custom implementations. They controlled the client after all.
Custom IDs require Custom clients.

Using a custom ID without changing the client I think *would* work since the server has the final say on things like that I believe, but the client would definitely not display it properly I'm almost certain

The client wouldn't know what to display and the player can't click anything.
