now i have a small error i have to take a look, so it seems it wasn't related to spells i think? Beacuse im using with the char the spell 31961 but the debuff occurs only when u are in combat, now i have to take a look in that (I made the insert into spells_dbc, copied all the data from the spell but tested didn't tested it without the insert)

I have no idea if it's possible to do by default and because of that I can't tell you how to do it. If it's not possible by default, I guess having 1 creature per quest and use CanBeSeen to phase all but 1 out for each player and randomize which one for different players.
