you could use the same flag to set the state of the bonfire (gameobject) in case it despawn/spawns (for whatever reason. AC currently never unloads grids so not sure how that would happen but still )

I've seen one that wasn't. I can't remember where and it was probably just a bug somewhere. Note: I'm the only one that plays on my realms so it wasn't another player that put it out
