Checklist
1) ensure your acore_auth.realmlist table is configured correctly. Your public IP (reported back by websites like whatismyip) should be the address column.
2) ensure you are editing the Data tab in heidisql and not the Table tab. Otherwise you're not actually changing the realm address. You can check this by seeing what ip your authserver.exe reports back when it starts up and adds the realm.
3) Ensure your ports are forwarded correctly. You need 3724 and 8085 forwarded. They should be forwarded for the internal ip of the machine hosting the server.
4) ensure the firewall has rules to allow traffic on those ports.
5) ensure your friend is setting the correct ip in their realmlist.wtf
6) ensure your friend is editing realmlist.wtf and not config.wtf.
7) ensure their realmlist.wtf file is being properly read from. After editing it and saving, when they launch wow it will update their config.wtf file with the realmlist from their realmlist.wtf file. If this does not happen, then make sure their realmlist.wtf doesn't have a hidden .txt extension. Also make sure it isn't in read only mode. Both of these can be checked in the file properties.

For some reason most people here have routers that do not support loopback so it's very likely that it wouldn't work
