progression levels. you start out at 0. When you try to go through the dark portal your progression level is checked. if you do not have the required level, you're not allowed through. This way NPCs and game objects are hidden / shown as well. Your progression level goes up after defeating certain bosses or completing certain quests. Other changes, like item changes or loot changes, are applied directly to the database.
