Starter gear is from DBC file CharStartOutfit.dbc and can be overriden by importing this file to SQL and importing to your world DB's table charstartoutfit_dbc

The spawn itself wouldn't be too hard. But you might want to change more so low levels have a couple of quests, too. I'm not sure if a lvl 1 paladin bloodelf has sufficient quests and a trainer e.g. in the valley of trials.

Okay i have no idea, i tried looking the guides and wiki which only setup i feel, could be cool with some editing tutorials as well
