It's been ages since I died from it. Usually it was because I used auto run and went AFK running against the door and when the lift came up I glitched through the lift itself because the door opened and I was rubbing against it.

If you dont care about the immersion flying on original map looked fine to me, but you need to patch client too or use dumb workarounds if you dont want to patch client

There are 4 major cities per faction and the "main" one has changed over time. In vanilla I remember Ironforge for Alliance and then it changed to Stormwind for some reason.
