You've to know what data was moved from creature_template to creature_template_model.
So you need to know
ID, Index, ModelID, Scale, Chance and theres another one.
You will use the same Index of the creature_template.
And use 0 to 3 (which in the old table it goes from 1 to 4)
And i think you've to grab the scale.
0 = modelid1
1 = modelid 2
2 = modelid 3
3 = modelid 4
Your npc only should have modelid1
