I can't say how to fix it but I can confirm it happening **a lot**. Naxxramas is another place where it happens very frequently.

I have seen the unit frame have red around it, sometimes yellow/orange, so something is aggroed but won't reset/show itself or something.

I don't know if a threat meter could tell you what is angry with you, I mean to check that out to see if I can determine what is actually keeping me in combat but I haven't played since I thought of it so I haven't yet.
