The icon of items are loaded from the DBC so if you create a custom item it will only have a red question mark as the icon unless you edit the DBC, yeah. Probably other side effects too but I don't know, I don't do custom items myself. Spells are also in a DBC and new spells need to be added to that.

Question when i load a moduel and recompile everything i notice i get a new auth and world server configs in my build folder do i use the new ones or can i just forget about them

If you mean that you originally renamed the authserver/worldserver.conf.dist to just .conf - yes. They'll always be created when building succeeds. Of course if an update comes out that adds/removes options those won't be automatically added to the .conf, only the .dist.

ARAC isn't a module in the traditional sense. It's just DBC and SQL.

It's one of the few that's not a "traditional" module. It doesn't have to be added to the core and compiled. As long as you place the DBCs in the right place (server- and client-side) and import the SQL file to the database that's all it needs.
