it's rare because they are not on the same timer.
the cycle for the second moon is longer than 24h.
just had second moon and sun up for example.
but you can have both moons up as well if the stars, or rather moons (badum-ts) align

dunno if it can be fixed in the exe with a simple ASM patch... a simple memory edit once the app runs should be possible for sure though, that's what i did as a quick test.
