if I make a custom item is there any way to have it have a real icon when it's in player inventory? I just get a red question mark and I recall reading that you have to mod the client to support that?

Needs data in item.dbc, if you store DBC data in mysql you can automatically generate that

You can spin up the idea a little bit more with implementing SQL updates over files like trinitycore and only store changes in the database like blizz hotfixes
That makes handling db2's and DBC soooo much easier
