Yes, you have to re-extract updated map files

You also have to update MapDifficulty.dbc and instance_template DB table (or along those lines) if it is a dungeon

your patch data should be in patch-4.MPQ, and your DBC's also in patch-Data\enUS\enUS-4.mpq
