up until Cataclysm, creatures were sent as separate entries for each difficulty. This is no longer the case with later expansions. They all use the same creature id but send different cache data based on difficulty. So creature_template_difficulty has been introduced to store said data.

(this rule also applies to modern classic except that they have blocked bruteforcing that difficulty based data)
