3 dimensional and 2 dimensional.
3 dimensional takes height into account, while 2 dimensional only takes X and Y into account

Yes. Imagine yourself flying right above a npc at a height of 10m.
2d distance would be 0 (because your x and y values are the same), but 3d distance would be 10.

only goes up to a maximum of 100 yards because you have to click on the NPC that is further away and the visibility is limited

exact = the exact distance between the points (that is what you want)
the others are from npc contact point to npc contact point
