Personally, I've never seen an item I wasn't able to look up on WoWhead or other online databases.
Maybe the item got a different ID entry later?

You mean, you want to know how to get the item to display the correct icon, not a question mark?
That requires a patch where you edit a .dbc-file.

Okay, you mean an item that is in the item.dbc, but not in the database?
Looked it up, and indeed: there is an entry for an item with 3579 in the `item.dbc`, but without an entry in the `world.item_template`.
Maybe someone else knows exactly why this is. Wild guess from me would be that the item got removed from the database at some point, but it remained in the `item.dbc` because some DEV was too lazy to remove it from the `item.dbc` as well? ♂️
