Maybe I'm incorrect, but based off all the videos and wowhead their HP doesn't actually go down. I think it's a timer system based on which NPC it is that actually spawns. You've got Injured Soldier, Badly Injured Soldier, and Critically injured soldier.

This video is WoW Classic TBC, and I didn't see their healthbars moving. Weird

If you're playing World of Warcraft The Burning Crusade Classic and you're Horde faction and want to unlock first aid skill to 300, just be sure u've lvled it up and then go to Arathi Highlands. There you can accept the Quest Triage.
