it's really strange, because when I start with `:d` and run `docker compose ps` there is only the database container

the original developer for the `docker-compose.yml` used compose profiles extensively. I personally think it makes things a bit more complex but at the end of the day it still works.
to see it properly you can run `docker compose --profile app ps`
