I want to add a second realm for testing purposes. Keep same table to login but seperate world/character database. Do just run the create sql script with different names for world/char dB and adjust this in the world config of the second realm?

You shouldn't need to run that script at all for the second realm. Just specify different char/world db names in the worldserver's config and it will prompt you to create the DBs at startup.
