It's an issue that occurs every now and then. You have to force an update by accepting/turning in a quest or log out and back in. I'm not sure if it's being worked on right now but I know several people here know about it so it's at least confirmed.

Well, I know several users here have confirmed it but I haven't looked if there's an issue report for it yet - but there should be. I might create one when I have the time to spare, but it's something with quest updates not triggering or something.

If I ever get more comfortable with the core, I'll try to assist with fixing it.

I think I might have found a way to consistently trigger it at least. It's happened every time I've tried to do the temple of the moon quest

That's interesting. It's very random for me, traveling somewhere has a chance of not showing any quests at all unless I log out and back in, but if a creature has gossip options set so you can talk to them even without quests you can talk to them and accept the quest and that'll force an update. Accepting a quest, going out and completing it and returning also has a chance of not showing the **?** when I return.
